Internal Revenue Service regulations require that certain companies and individuals be provided with 1099 forms. In order to accurately prepare these forms, the IRS requires that we obtain and maintain form W-9 for all of our vendors. We ask that you complete and submit the online Substitute Form W-9. The information return must disclose the vendor’s taxpayer identification number. Failure to provide your Social Security Number or EIN may result in the delay of future payments or backup withholdings.

Below are general instructions for completing the online Substitute Form W-9:

  1. Open the Substitute W-9 template by clicking "W-9 Form" button below.
  2. Enter your name, business name, and address in the corresponding boxes.
  3. Check the appropriate box for the federal tax classification (whether you’re a sole proprietor/single-member LLC, a C corporation, S Corporation, in a Partnership or a Trust/Estate).
  4. Fill out your Social Security Number or Employer Identification Number.
  5. Check the appropriate box for the type of organization in which your business is conducted.
  6. Sign and date the form, click ‘Done’ to submit the W-9 form. This form will automatically be submitted to MPAC when you click the "Done" button.
